How to add diagnosis codes to your custom forms

Please ensure that you have either chosen a new custom form evaluation or are currently in the process of editing one: 

  1. Click on ‘Smart components’ and select ‘Patient(s) Diagnosis Codes’.

2. Label your diagnosis code component and click on ‘Accept’ to add the component to the custom form.

3. The diagnosis code component consists of a dropdown with all the diagnosis codes, radio buttons to specify the type of diagnosis code and a button to add new diagnosis codes.

Organizations can now change their settings pertaining to diagnosis codes. To do this ensure you are logged in to Ambiki and on the main dashboard:  

  1. Click on ‘Organization menu’ and select ‘Edit organization’.

2. Click on 'EMR settings’.

3. Modify your diagnosis codes settings by checking or unchecking the checkboxes labelled “ICD-10 diagnosis codes should be marked as 'suspected' unless they are derived from an evaluation or re-evaluation.” and “After an initial evaluation, any future ICD-10 diagnosis code changes can only come from a re-evaluation.”
