How to create an Evaluation? 

To create an Evaluation, ensure you are signed in to Ambiki and on the main dashboard: 

 







2. Select the patient that you would like to create the evaluation for by clicking on their name. 

 

 

 






3. Towards the left side of the page, click on Evaluations. 

 

4. On the top right-hand corner, click on Add an evaluation or Add an external Evaluation.

 


 

  

 

 




 5. Fill in all the details of the evaluation as needed. Depending on your practice settings, you may see a green button in the top right-hand corner that allows you to change the evaluation template. 

 

 

 





6. On the bottom right-hand corner click on Save evaluation.
